Alzheimer's is a disease that most of us know as forgetfulness. But does anyone who suffers from amnesia have Alzheimer's? Alzheimer's is a progressive brain disease that affects a large population in the world. The destructive course of this disease and its costs are increasing every year. It is estimated that 24 million people worldwide are affected by this disease and they account for 84% of all deaths. This year is the 100th year since this disease was discovered. In relation to this disease, its causes and factors, as well as its prevention and treatment solutions, we have arranged an interview with Dr. Hamidreza Torabi, a specialist in brain and nerve diseases, which we will read together.
What is dementia and what are its symptoms?
In dementia disease, intellectual abilities and mental and cognitive abilities decrease. All Alzheimer's patients have dementia, but every patient who has dementia does not necessarily have Alzheimer's. In fact, Alzheimer's is one of the diseases of dementia.
What causes dementia?

Other causes of irreversible dementia are Parkinson's disease with dementia, AIDS, advanced MS and multiple vascular lesions and multiple strokes.
What is the difference between normal forgetfulness in old age and Alzheimer's?
Alzheimer's in old age has special characteristics that we can diagnose this disease by paraclinical observations including patient examination, CT scan and MRI. In the case of elderly patients who have amnesia, it is possible to state causes such as depression. However, with careful examinations and tests, brain imaging, and especially the use of new diagnostic techniques, it is possible to find out the presence of Alzheimer's disease in a patient. Currently, the new diagnostic method that has just arrived in Iran is to perform "Volumetric MRI" or measure the volume of the brain. This method shows the decrease in the volume of brain cells, which confirm the presence of Alzheimer's disease. Other diagnostic methods are the use of intelligence tests and psychological tests, which justify the presence of Alzheimer's or vice versa, depression in old age in these people.
Does Alzheimer's affect the patient's short-term memory more or his old and long-term memory?
Initially, when the patient visits the clinic, the first sign of forgetfulness appears mostly in his near and short-term memory, in the way that the patient leaves an object in his room, and constantly asks the people around him to find that object again. But gradually, the long-term and remote memory of these people also becomes problematic, so that after a while these patients even forget the names of their children and have no knowledge of time and place. Impairment in logic, judgment, ability to calculate, newness, difficulty in speaking, lack of understanding and even lack of stool and urine control gradually comes to these patients.
What is the approximate age of onset of this disease and does it affect women or men more?
This disease starts around the age of 60-70. Of course, the onset of Alzheimer's under the age of 60 and over the age of 70 has also been reported. Alzheimer's is seen in both women and men in the same proportion.
What are the most known causes of Alzheimer's?
In general, no specific cause has been proposed for this disease. Although there are a series of tissue changes in the genetics of the family members of these patients, chromosomal changes that lead to Alzheimer's disease are not mentioned, so Alzheimer's does not have a hereditary cause, and no other cause has been mentioned so far. The hypothesis of the effect of aluminum in drinking water or water pipes or some containers in causing Alzheimer's disease has not been proven yet.
What measures does nursing care for Alzheimer's patients at home and in nursing homes include?
In this regard, the first step is to justify the patient as well as those around him about the existence of the disease. Because the lack of awareness of the surrounding people about the existence of the disease causes many problems for Alzheimer's patients. The second measure is the health issues of these patients because these people cannot take care of their health issues in the advanced stages of the disease. Eating healthier is one of the most important measures in nursing these patients, because overeating with excessive undereating and malnutrition puts these people at risk. In advanced stages, due to the increase in hallucinations and delusions, Alzheimer's may cause problems for family members, including children. It is recommended to transfer the patient to the hospice when he/she is no longer aware of the people around him, although with the consent of the family and friends, these people should be transferred to the hospice for several reasons, because the patient is monitored in the hospice environment if he/she is restless. Nursing measures in the sanatorium also include physical therapy - occupational therapy, body massage and taking medication on time.
To what extent does Alzheimer's cause disorders in affected people, and in this case, is behavioral therapy effective in them?
The presence of personality disorders in these people is undeniable. Due to the lack of knowledge in judgment and the lack of cognitive power, Alzheimer's patients gradually experience hallucinations and delusions and may cause serious harm to themselves and others. Depression and anxiety are also among the psychological disorders that appear in this disease. Regarding the effect of behavioral therapy on these people, it should be said that since they have no knowledge about their disease, behavioral therapy cannot be effective. But psychological interventions are very important for both the patient and his family. Individual and family observation also delay the hospitalization of patients in the hospital for almost a year.
Who is less likely to develop Alzheimer's disease?
Most educated people and people who study daily and follow a healthy diet, experience little stress and use a series of vitamins such as B12 and oxidants, are less likely to get Alzheimer's in the future than others.
What are the latest approved and effective treatment methods for these patients?
Alzheimer does not have a specific treatment that can stop the disease process in general. The treatment of these people is mostly in the form of supportive treatments and delays in the progress of the disease. The prescription of BEC vitamins and folic acid by the doctor in appropriate doses and physical and nutritional health should be considered. Currently, the newest drugs considered and available in the world for these patients include Memantine, Exelon, Aricept, Reminil.
