The surgical stage is performed in two ways 1) Open or flap surgery After examining 3D CBCT radiographs The gum is cut (flap) and with special drills, a hole is created inside the jaw bone, then the implant screw, which is made of titanium, is inserted into the bone and the soft tissue (gum) is sutured. If the amount of bone is not enough, bone grafting may also be done. Photo 1 2" digital surgery (guided implant placement) In this surgery, 3D photos and intraoral scan are prepared before performing the work, and the exact position of the implant is evaluated in the special software, and the distance of the implant from the nerve and anatomical areas is determined very precisely, as well as the length and diameter of the implant and the need for bone grafting. According to the final position of the implant, a guid or resin surgical guide is made with a 3D printer, and for each implant, a channel is built into it, which is used like a template to guide the surgical path. This surgical guide helps us to perform drilling and placing the fixture (screw implant) often without flapping and opening the gum and without the need to see the bone directly with the surgical cam guide and to complete the surgery without the need for stitches, this type of implant implantation is called the punch or flapless method. But sometimes it is not possible to perform the surgery without incision and it is necessary to perform implant implantation with a minimal incision (Minimally Invasive Flap). In the digital implant implantation method, the type of implant and its brand are no different from the normal method, but the surgery is very precise and the bleeding is much less, since there is no cut or suture, the pain after the operation is much less than the open method. With the presence of technology, there is no need to make deep and large incisions on the gums, and a large percentage of patients will no longer need traditional methods. Digital method features: 1\. Smaller incision and minimal flap, fewer sutures needed Usually a small incision is made on the gum instead of a wide flap. 2\. Maintaining blood supply: reducing the amount of gum and bone manipulation helps maintain blood flow and heal faster. 3\. Reduction of pain and swelling: due to speeding up the surgical process and the lack of bone opening during surgery, the patient will have a better experience during the recovery period. 4\. High accuracy with the surgical guide: due to the precise design of the surgical guide, the implant location can be determined more accurately. 5\. Possibility of immediate loading (Immediate Loading): In some situations, this method allows the placement of a temporary cover on the implant in the same session. This method is especially ideal for patients who have good bone density and do not need to increase bone height or width. Photo 2 Prosthetic stage (implant cover) After 2 to 6 months after the surgery, when the implant is integrated with the jaw bone (welded), it is done. Photo 3 which is done in two ways: 1. Traditional method (Conventional Impression): ✅ In this method, molding is done inside the mouth with silicone or polyether materials and the mold is sent to the laboratory for further work. ✅ Using a mold, the laboratory prepares a plaster model and designs and manufactures the coating on the abutment (abutment or implant base) and then sends it to be placed in the patient's mouth. Disadvantages of the traditional method: ❌ Deformation of molds due to sensitivity to humidity, temperature and time, which can cause minor changes in the final accuracy of the coating. ❌ The patient's feeling of discomfort during molding, especially in patients with a high gag reflex or people who have shortness of breath. ❌ The process is time-consuming: the need to send the mold to the laboratory and return it, which may take several days to several weeks. 2. Digital method (Digital Impression): In this method, molding is done using an intraoral scanner instead of materials. Digital data is sent directly to the digital laboratory or CAD/CAM devices and the cover is made using 3D Milling technology or a 3D printer. Advantages of the digital method: ✅ Very high accuracy: eliminating the error of changing the shape of the molds and increasing the compatibility of the cover with the implant. More comfort for the patient: without the need for molding materials, the patient will have a better experience. Reduced manufacturing time: digital information is sent immediately and in some cases veneers can be made and delivered on the same day (Chairside CAD/CAM). ✅ The possibility of more accurate design: digital software provides the possibility of designing and modifying the cover before manufacturing. ✔ The traditional method is still used, but in some cases it makes the patient uncomfortable and reduces accuracy. ✔ The digital method is a better choice for patients and dentists due to its higher accuracy, convenience and speed.
