Introduction:
When talking about superficial tumors of the skull skin, many patients feel anxious; But the fact is that most of these lesions are benign and with early diagnosis and appropriate approach, they often have very good results. Like the guard that guards the doors of our body, the skin of the skull is also our first line of defense against environmental factors and sun damage. In this scientific but humane article, I intend to examine the common and rare types of these tumors with a view from the heart of experience and science, clarify the stages of diagnosis and treatment, and accompany the reader in the path of informed decision-making.

Common types of superficial skull skin tumors

1. Benign lesions

  • Pilar Cyst (Trichilemmal / Pilar Cyst): It accounts for about 40-50% of benign tumors of the skull skin. These cysts are usually soft, well-drained and asymptomatic. DermNet®
  • Lipoma and melanocytic nevus: Among other benign cases, lipomas account for approximately 30% and melanocytic nevus for approximately 28%. DermNet®
  • Actinic & Seborrheic Keratoses: With increasing age and decreasing hair coverage, these lesions become more common; Actinic keratosis can develop into SCC. DermNet®Wikipedia+1
  • Skin osteoma (Osteoma cutis): There are small and hard nodules of bone origin in the skin layer, and sometimes non-invasive treatment such as tetadoine can be effective. Wikipedia

2. Malignant tumors

  • Basal cell carcinoma (BCC): The most common skin cancer, especially in brighter areas; It often has a pearly or ulcerated appearance and can be treated with open surgery or superficial treatments. HealthlineCleveland ClinicVerywell Health
  • Squamous cell carcinoma (SCC): the second most common skin cancer; It usually appears as a resistant ulcer or sloughed lesions. Healthline Health
  • Melanoma: Rare but dangerous, with a high spread rate. It is less on the scalp than other areas, but knowing the appearance of ABCDE is vital. Healthline Health
  • Other rare tumors (Adnexal tumors, Merkel cell carcinoma, Kaposi sarcoma, etc.): Depending on the clinical features, they can be distinguished by biopsy. Barrow Neurological Institute noopener">DermNet®

Diagnosis, treatment and way forward

Accurate diagnosis is based on clinical examination, biopsy and medical images. Like steering a ship in a stormy night, you should proceed carefully in the treatment process:
  • Diagnosis and Biopsy: Any new or changing lesion requires careful evaluation. For suspicious tumors, a biopsy is gold. Verywell Health
  • Treatment methods:
    • For primary BCC and SCC: local surgery, procedures such as Mohs or skin grafts after resection. PMCLippincott Journals
    • Topical treatments such as Efudex (5-FU) for superficial and accessible lesions. Verywell Health
    • Melanoma requires diagnostic expansion, imaging and sometimes systemic treatments.
  • Prevention and follow-up care: Use of hats and sunscreen, skin examination training and regular visits by a specialist. Barrow Neurological Institute

A bridge of experience to science

I remember one day in Tehran, I was visiting a teenager who had a small lesion on his skull. It was less than a millimeter, but with a little carelessness it could have turned into SCC. We treated with thin slits of 5-FU cream (Efudex) and after a few weeks, the lesion disappeared; No wounds and no worries. These small clinical experiences teach us that "prevention is better than cure", even if the lesion seems small and unnoticed.

If you have a lesion on your scalp or have questions about a condition such as a superficial scalp tumor, I recommend that you get it checked out by a dermatologist first.
